Pharmacy Profiles’ mission is to serve as a trusted and verified repository of the nation’s pharmacist providers, to enable the nation’s pharmacists, student pharmacists, pharmacy technicians, and other users to manage their professional information all in one place, and to provide a single source for employers, health plans, payers, pharmacy networks, government, and others to validate professional information about pharmacists, student pharmacists and others. Central to this mission is our commitment to be transparent with you about the data we collect about you and how it is used and shared.

By using our Services, you consent to our use of your data under this Privacy Policy.

1 Introduction

We are a platform for the nation’s pharmacy professionals and a subsidiary of the American Pharmacists Association. Registered pharmacists, student pharmacists, pharmacy technicians, and other types of users of our Services are “Individual Members.” Individuals designated by a pharmacy or pharmacy organization to manage pharmacy-related data and tagging pharmacist, student pharmacy and pharmacy technician employees are “Organization Members”. Pharmacist employers, health plans, payers, schools of pharmacy and others who contract for Services from us are “Customers.” Other people accessing or visiting our site are “Visitors.” This Privacy Policy applies to any Individual or Organization Member, Customer, or Visitor to our site.

1.1 Services

This Privacy Policy applies to PharmacyProfiles.com and to services provided by Pharmacy Profiles through and as part of our website and associated applications (“Services”). If you use our Services or access our site, you consent to this Privacy Policy.

1.2 Participation

Participation in Pharmacy Profiles is voluntary and does not replace your continuing education credits being made available to the state boards of pharmacy that you have designated in CPE Monitor for re-licensure reporting.

1.3 Consent

If you use our Services, you consent to the collection, use and sharing of your professional data, and any relevant personal data, under this Privacy Policy and agree to our User Agreement.If you use our Services after an update to this Privacy Policy, you consent to the changed policy.

1.4 Change

We may modify this Privacy Policy and our User Agreement from time to time. If we make material changes to either, we will provide you notice on our site or as part of our Services to you, or by other means, to provide you the opportunity to review the changes. If you object to any changes, you may close your account. Your continued use of our Services after we publish or send a notice about our changes to these terms means that you are consenting to the updated terms.

2 Information We Collect

2.1 Information You Provide to Us

You provide professional data, and certain personal data, to create an account with us.

2.1.1 Pharmacy Profiles® Registration

You provide professional data, and certain personal data, to create an account with Pharmacy Profiles.

To create an account as an Individual Member, you provide data that may include your name, email address, NABP (National Association of Boards of Pharmacy) eProfile identification number, your birthdate, Pharmacy College Application Service (PharmCAS) identification number, and a password. If you register for a premium Service, we may ask you for payment (e.g., credit card) and billing information. You create your Pharmacy Profiles Profile. A complete Profile helps you get the most from our Services.

To create an account as an Organization Member, you provide data that may include your name, email address, and a password.

To create accounts as a Customer, the Customer organization provides the first and last name, and email address, of the person or people in your organization who will be responsible for or have access to your account,. Each Customer user will create their individual account with a username and password. Customer Organizations are responsible for managing access for their individual users, including promptly removing access when a user leaves the organization.

2.1.2 Sensitive PII and Social Security Numbers

While using general services as a Member does not require it, if you choose to enroll in specific Services such as credentialing application or roster enrollment, we will explicitly request your Social Security Number (SSN) and/or other highly sensitive Personal Identifiable Information (PII) necessary to fulfill the specific services.

2.2 Information/Data We Collect and Integrate from Third Parties

Pharmacy Profiles populates your Profile with information from organizations that may include the Accreditation Council for Pharmacy Education (ACPE), American Pharmacists Association (APhA), National Association of Boards of Pharmacy (NABP), National Council of Prescription Drug Programs (NCPDP), American Association of Colleges of Pharmacy (AACP), education organizations, credentialing organizations, and schools of pharmacy.

ACPE, APhA, NABP, NCPDP, AACP and others transferring from their systems appropriate information including, for example, your birthdate, to Pharmacy Profiles to assist it in populating your Profile. The general information transferred to Pharmacy Profiles to populate your Profile will NOT include credit card numbers. However, if you choose to participate in or use specific services that legally or operationally require identity verification, credentialing, or enrollment rosters, you will be prompted to explicitly provide your full Social Security Number (SSN). When provided, your SSN is strictly governed by the enhanced security and privacy protocols detailed in Section 3 of the User Agreement.

Pharmacy Profiles populates your Profile with other professional information from other publicly available or third-party sources.

An employer, school of pharmacy, pharmacy network, or other type of Customer may provide us information including demographics, credentials and qualifications of Members they employ, enroll or otherwise are affiliated with for users who are Members and those who are not Members.

We encourage you to visit your Profile often and keep it up to date. Please also review your privacy and other settings, as they will affect what information may be made visible or available to employers, health plans, and other users of Pharmacy Profiles’ services.

2.3 Service Use

We log usage data when you visit or otherwise use our Services. We use logins, browser information, device information and internet protocol (“IP”) addresses to identify you and log your use.

2.4 Cookie Policy

2.4.1 Transparency and Data Minimization

Pharmacy Profiles uses cookies, web beacons, and similar tracking technologies to ensure the security, reliability, and functionality of our Services. We are committed to data minimization; our cookies are designed to collect only the metadata necessary to authenticate users, maintain secure sessions, and monitor system performance. We do not use cookies to collect, store, or transmit Sensitive PII, such as Social Security Numbers.

2.4.2 Categories of Cookies We Use

To provide a transparent understanding of our data processing, we categorize our cookies as follows:

  • Strictly Necessary & Security Cookies (Required): These cookies are essential for the core operation of Pharmacy Profiles. They enable secure login authentication, session management, and protection against unauthorized access or cross-site request forgery (CSRF) attacks. Because these are critical to the security and functioning of the platform, they cannot be disabled.
  • Functional Cookies: These cookies allow our Services to remember choices you make (such as your username or regional preferences) and provide enhanced, more personalized features. They only last for as long as your session (usually the current visit to a website or a browser session).
  • Performance and Analytics Cookies: These cookies collect aggregated, anonymous information about how Visitors and Members use our site (e.g., which pages are visited most often, or if users receive error messages). This data is strictly used to improve the performance and user experience of our Services.

2.4.3 Your Choices and Control for Cookies

While Strictly Necessary cookies cannot be disabled without compromising the security of your account, you have the right to manage or disable non-essential cookies. You can modify your cookie preferences at any time through your browser settings. Please note that disabling Functional cookies may limit your ability to use certain features of our Services.

2.5 Your Devices and Your Location

We receive data from your devices and networks, including location data.

When you visit or leave our Services, we receive the URL of both the site you came from and the one you go to next. We also get information about your IP address, proxy server, operating system, web browser and add-ons, device identifier and features, and/or ISP or mobile carrier.

2.6 Other

We are continually updating and improving our Services, which means we may add or ask for new/additional data and develop new ways to use data.

Our Services are dynamic. We often introduce new features, which may require the collection of new information. If we collect materially different professional or relevant personal data or materially change how we use your data, we will notify you and, as necessary, may also modify this Privacy Policy.

3 How We Use Your Data

We use the data that we have about you to provide, support, personalize and make our Services more relevant and useful to you and others. Pharmacy Profiles works to:

  • Serve as a trusted and verified repository of the nation’s pharmacist professionals,
  • Enable the nation’s pharmacists, student pharmacists, pharmacy technicians, and other users to manage their professional information all in one place, and;
  • Provide a single source of professional information about the qualifications and credentials of pharmacists, student pharmacists, pharmacy technicians, and others to employers, health plans, payers, governments, schools of pharmacy and others with which these Members are affiliated. These organizations use this information to confirm the completion of requirements and to determine eligibility to provide patient care services, credentialing and enrollment with health plan networks, and other activities.

The Pharmacy Profiles platform is also the infrastructure for the American Pharmacists Association ADVANCE® professional development service. This partnership fuels the single sign-on between ADVANCE® and Pharmacy Profiles and the flow of certain information between Pharmacy Profiles and ADVANCE platforms for users that use both services.

3.1 Services

Our Services help Individual Members, Organization Members and Customers by presenting to employers, health plans, payers, governments, and others information to validate professional information about pharmacists and determine their eligibility to provide patient care services.

We use your data to authenticate you and authorize access to our Services.

3.2 Customer Support

We use data about you and your activity while using our Services to help you and fix support issues you may have.

We also use data to investigate, respond to, and resolve complaints and Service issues (e.g., bugs).

3.3 Security

We use your data if we think it’s necessary for security purposes or to investigate possible violations of our User Agreement or Privacy Policy.

3.4 Credentials Verification, Credentialing, and Background Checks

When you provide your SSN or other Sensitive PII, we use this data strictly for identity and qualifications verification, background checks, and professional credentialing as required by the specific services you have requested. This process may be subject to the Fair Credit Reporting Act (FCRA) when utilizing third-party Consumer Reporting Agencies.

4 How We Share Information

4.1 With Customers

We will share with employers, schools of pharmacy, health plans and other organizations who are Customers selected demographic and professional information, such as education and license information, state and federal sanctions, advanced credentials and certifications, and selected CPE.

4.2 Sensitive Personally Identifiable Information (PII), including Social Security Number (SSN)

We do not publicly display your SSN on your Pharmacy Profiles Profile. We will only share your SSN, PII, or related background check results with authorized third parties, credentialing agencies, or specific Customers when you explicitly authorize us to do so as part of a requested service.

5 Your Rights and Controls

5.1 Rights to Access and Update Your Data

You can continually access and edit/update your data at any time. Some data elements provided by authoritative, external primary sources may require you to exit the Pharmacy Profiles Site, Service or Application and update the data at the Primary Source directly. Some updates with external providers may take additional time to refresh in the Pharmacy Profiles Site, Service or Application after changes are made. The additional time to refresh is determined by the data sharing and SLA agreements between Pharmacy Profiles and the individual external primary source data providers.

5.2 Rights and Controls for Data Sharing Authorization

You have the right to opt-out, change or revoke your authorization for Pharmacy Profiles to share your data at any time. You can make these changes in the Data Sharing Settings for your account on the Pharmacy Profiles Site.

5.3 FCRA Disclosures and Rights

For services involving background checks, you have specific rights under the Fair Credit Reporting Act (FCRA), including the right to be told if information in your file has been used against you, the right to know what is in your file, and the right to dispute incomplete or inaccurate information. 6 Security

6.1 Shared Responsibility

Pharmacy Profiles utilizes industry-standard security protocols and continuous system monitoring to defend your data against unauthorized access. Securing your information is a shared responsibility, and we strongly advise you to utilize the security tools and features available within your account settings.

6.2 Sensitive PII (including SSN) Encryption

For the protection of Sensitive PII, including your Social Security Number, Pharmacy Profiles maintains comprehensive, industry-standard administrative, technical, and physical safeguards. Specifically, highly sensitive data such as SSNs are encrypted both in transit and at rest.

6.3 Sensitive PII (including SSN) Internal Access

Access to Sensitive PII data, including your Social Security Number, within Pharmacy Profiles is strictly limited to authorized personnel on a ‘least privilege’ basis necessary to perform their job functions.

6.4 Data Retention

We retain your personal data for as long as your account is active or as needed to provide you Services. Sensitive PII, such as SSNs collected for specific credentialing events, is retained only for as long as legally or operationally necessary to fulfill that purpose, after which it is securely deleted or anonymized in accordance with our data destruction policies and applicable laws.

6.5 No Guarantee of Absolute Security

While we implement robust security measures designed to protect your data, no method of transmission over the internet or electronic storage is 100% secure. Therefore, we cannot guarantee the absolute security of any information you send us, nor can we warrant that data will not be accessed, disclosed, altered, or destroyed in the event of a breach of our physical, technical, or managerial safeguards.

6.6 Data Breach Notification

In the event of a security incident that compromises the confidentiality, integrity, or availability of your Sensitive PII (including your Social Security Number), Pharmacy Profiles will act promptly to mitigate the risk and secure our systems. We comply with all applicable state and federal data breach notification laws.

If we determine that your personal information has been subject to unauthorized access or acquisition that poses a reasonable risk of harm, we will notify you without unreasonable delay. This notification will include:

  • A description of the incident and the specific data involved.
  • The steps we have taken to contain the breach and protect your data.
  • Actionable advice on how you can protect yourself from identity theft or fraud.
  • Information regarding complimentary credit monitoring services, if legally required or deemed appropriate based on the nature of the compromised data.
