Background Screening Services
Pharmacy Profiles offers professional credentials verification and credentialing application services to employers, health plans, and other organizations that need to understand the credentials of pharmacy professionals. Pharmacy Profiles does not currently provide these services directly to pharmacy professionals. These services may be considered background checking services under federal and state law. Although Pharmacy Profiles does not conduct typical background screenings, such as credit and criminal checks, we comply with the federal Fair Credit Reporting Act (FCRA) and similar state laws in performing these licensure and credential verification services.
Pharmacy Profiles collects information from pharmacy professionals and other sources to verify professional licensure, registration, credentials, pharmacy education, employment data, and regulatory or law enforcement discipline or sanctions. Under the law, these services are known as investigative consumer reporting services. When providing background checking services, all the information gathered about pharmacy professionals is collected and used as permitted and in compliance with the FCRA, 15 U.S.C. § 1681 et seq., as well similar state laws, including the California Investigative Consumer Reporting Agency Act (ICRAA). These laws regulate how background checking services may be conducted by Pharmacy Profiles.
The FCRA and similar state laws were enacted to protect the information collected when performing background checks. They also protect pharmacy professionals by promoting the accuracy of the information and the privacy of pharmacy personnel by regulating the disclosure of information.

FCRA Summary of Rights
The federal FCRA promotes the accuracy, fairness, and privacy of information in the files of consumer reporting agencies. There are many types of consumer reporting agencies, including credit bureaus and specialty agencies (such as agencies that sell information about check writing histories, medical records, and rental history records). FCRA Dispute Process
Pharmacy Profiles is committed to maintaining the accuracy and completeness of the information in our credentials verification service and credentialing application reports (background screening reports). Pharmacy Profiles prepares reports as part of its investigative background checking services detailed below. In accordance with the FCRA, we have established the following procedure (Procedure) if a pharmacy professional believes that the report containing their data has inaccurate or incomplete information.
Dispute Resolution Procedure
This Procedure applies to information about pharmacy professionals that is collected or used in connection with investigative consumer reporting services (Services). These Services are commonly known as background checking services. Pharmacy Profiles offers Services to employers, employers, health plans, and other organizations that need to understand the credentials of pharmacy professionals. Pharmacy Profiles does not currently provide these services directly to pharmacy professionals. The Services include verification of professional licensure, registration, credentials, pharmacy education, employment data, and regulatory or law enforcement discipline or sanctions.
Procedure for Dispute
Pharmacy professionals can dispute the accuracy or completeness of information in Pharmacy Profiles reports by submitting the dispute form. The dispute form may be emailed to Pharmacy Profiles at dispute@pharmacyprofiles.com. If you have questions, contact Pharmacy Profiles by mail, phone, or email.

There is no charge for the investigation.
Upon receipt of your dispute form, Pharmacy Profiles will conduct an investigation and notify you of the results within 45 days. If an error is found, the inaccurate or incomplete information will be corrected, modified, or removed from the Pharmacy Profile report. If the dispute is not resolved to your satisfaction, you have the right to file a brief statement with Pharmacy Profiles summarizing the nature of your dispute regarding the accuracy or completeness of the information in the report.
