Pharmacy Profiles Investigative Consumer Reporting Privacy Policy

Pharmacy Profiles respects the privacy of the personal data of the pharmacy professionals contained in the Pharmacy Profiles system. We use encryption and other technologies and practices to secure the data. Our Investigative Consumer Reporting Privacy Policy (Policy) describes how we may collect, use, and safeguard pharmacy professional’s your personal information.

Overview

This Policy applies to information about you that is collected or used in connection with investigative consumer reporting services (Services). These Services are commonly known as background checking services. Pharmacy Profiles offers Services to employers, employers, health plans, and other organizations that need to understand the credentials of pharmacy professionals. Pharmacy Profiles does not currently provide these services directly to pharmacy professionals. The Services include verification of professional licensure, registration, credentials, pharmacy education, employment data, and regulatory or law enforcement discipline or sanctions.

Information Collected

Sources

Pharmacy Profiles may collect your personal data via any means, whether online, electronic, verbal, or written. We may collect information about you from your employer or prospective employer or an organization that may engage you as a contractor or volunteer. We may collect personal data from you. We may collect information about you from public sources, regulators such as a board of pharmacy or its website, or other sources such as a pharmacy technician certification organization. Your personal data may have been previously provided to or obtained by Pharmacy Profiles. For example, you may have already created a Pharmacist Profile and added personal data so you can track your professional information. Pharmacy Profiles may collect additional or clarifying information about you or maintain personal data as described in this Policy.

Types

The types of data about you (Personal Data) that may be collected or obtained, or that were previously collected or obtained, include, but are not limited to, the following:

  • First and last name (and middle name or suffix as applicable);
  • Address;
  • Email address;
  • Telephone number;
  • Date of birth
  • Educational data, including without limitation, the school or college of pharmacy from which you graduated, graduation date, internship hours, or continuing pharmacy education;
  • Professional information such as your license or registration, certification information, or licensure or registration history;
  • Employment-related data;
  • Regulatory or law enforcement-related information such as licensure discipline, or sanction, exclusion, or criminal data; or
  • Medical information that may be contained in the record of a disciplinary action or sanction published by a regulator or provided by a regulator to NABP or Pharmacy Profiles.

Use of Personal Data

Pharmacy Profiles uses Personal Data to process requests for background checks. To perform and process a background check, Pharmacy may need to share your Personal Data with third parties such as employers, potential employers, organizations that may engage you as a contractor or volunteer, regulators, law enforcement, data furnishers, and others as permitted by law.

Pharmacy Profiles may use your Personal Data to answer questions or communicate with you, employers, other organizations requesting a background check, or third parties assisting Pharmacy Profiles in providing the Services.

Personal Data is used as permitted and in compliance with the Fair Credit Reporting Act (FCRA) and related state laws.

Pharmacy Profiles may use your Personal Data as required by federal or state law, court orders, or legal process.

Security and Confidentiality

In the course of providing Services, Pharmacy Profiles staff may use or access Personal Data for the purposes stated in this Policy. Staff are required to maintain the confidentiality of Personal Data and are prohibited from unlawfully using or disclosing such Data.

Pharmacy Profiles protects the security and confidentiality of your Personal Data by requiring a login and password to provide Personal Data to Pharmacy Profiles and requires a login, password, and two-factor authentication to access background check reports online.

Secure socket layer and encryption technologies safeguard the transmission of Personal Data through the Pharmacy Profiles website. Internal and external firewalls are used to protect Personal Data held by Pharmacy Profiles. Additionally, administrative and physical security measures are in place to prevent unauthorized access to Personal Data.

Personal Data Disclosure: United States or Overseas

Pharmacy Profiles does not transfer Personal Data to third parties outside the United States or its territories for purposes of the Services. Personal Data submitted to Pharmacy Profiles for background investigations is not transferred outside of the United States. The collection and processing of Personal Data for background investigations are performed in the United States.

Compliance

Pharmacy Profiles adheres to the requirements of the FCRA and related state laws. For more information about the FCRA, read the Fair Credit Reporting Act published on this website. Updates to this Policy and Other Policies

Updates to this Policy will be posted on the website and will include the date that it was last updated.
Review the User Agreement which apply to your use of the Pharmacy Profiles website.

Conflict

If there is a conflict between the terms of this Investigative Consumer Reporting Privacy Policy and the Pharmacy Profiles Privacy Policy, the terms of this Investigative Consumer Reporting Privacy Policy control with respect to the Services. For all other services, products, or programs that Pharmacy Profiles offers, provides, or administers, the terms of the Pharmacy Profiles Privacy Policy control. For disputes related to the accuracy or completeness of your information in the credential verification service or credentialing application report (background screening report), complete the dispute form.
